2. The Patient Registration Forms work
best in a recent version of Adobe Acrobat Reader. To download a
free copy, click on the icon below... |

3. Once you have downloaded the
appropriate file below, you may either fill the registration forms out
on your computer screen and then print them out |

or |
| Print out
the registration forms and fill them out with pen and pencil. |
|
4. Select an
appropriate download shown below. When the download box comes up, click on
the "Run" button. In the final box, click on the "Finish" button. |

NOTE: If you no longer have access to "Owner" user name, replace "Owner"
in the destination box with your current user name (example:
C:\Users\Owner\Desktop to C:\Users\Newname\Desktop) |
